Kainé was a design baby who was born by mixing Kali and her husband's genes with Legion attack patterns. For the majority of her life, however, she was unaware of this and lived a sheltered life in Kali's luxurious mansion as Kali's granddaughter. With her grandmother strictly dictating her future, Kainé was raised as an obedient, soft-spoken, and formal maiden of high society. She spent her days wearing fine dresses in content isolation.

Her grandfather took pity on her and tried to bring joy into Kainé's life by giving her a white dog as a pet. Seeing as it is a female, Kainé names it "Shiro" (White) after Snow White.

During a Red Eye attack, Kainé secretly left her home to retrieve a runaway Shiro. She encountered Yonah, who was separated from her brother in the chaos of the attack, and brought the scared girl inside the mansion for safety. After Yura failed to destroy the Red Eye with P-22 (an earlier model of P-33), Kainé and Yonah are forced to flee from the burning mansion. She reunited Yonah with Nier and tried to go back to her grandparents.

Kainé returns in time to see her grandfather shielding Kali from a fatal blow from the Red Eye. He had a few parting words for Kainé, warning her that her world will forever change with the truth, but died when he was suddenly crushed by falling debris. A wounded Kali tried to finish what her husband was saying to the mourning Kainé but was devoured whole by the Red Eye before she can finish. In her grief and rage, Kainé unleashed her Legion powers (which she described as a "black power within her").

During her rampage, she ripped apart the Red Eye that devoured Kali with her bare hands but was driven berserk; she began attacking others around her after the Red Eye's death. Although the still-living Kali was recovered by the remaining Counter-measure against Special Biological Organisms Department soldiers, they were afraid of restraining Kainé and called her a monster. Nier defended her, insisting that she was a human who saved their lives. His pleas and belief in their humanity restored Kainé and Emil to their senses.

Sometime after the attack, Kainé was engaged to a man. The bug in the Replicant system resulted in Replicant Kainé. The booklet implies that Kainé's sturdiness and her swordsmanship in the game are derived from her Gestalt's genetics rather than Tyrann.
