Yonah, depending on the version of NieR, is either the younger sister or daughter of the original Nier, the Shadowlord.

White Chlorination Syndrome emerged in 2003 with the arrival of Caim, Angelus and the Queen-beast from Drakengard universe in Shinjuku, Tokyo. It was first propagated by the unnatural "maso" particles produced with the destruction of Queen-beast, and the disease appeared to be capable of airborne transmission.

Project Gestalt was initiated to save humanity from the incurable White Chlorination Syndrome. It was found that the spread and progression of WCS could be prevented by achieving Gestalt mode through separation of body from soul. As part of the project, surveillance androids were tasked to monitor the disease and prepare for its anticipated future extinction.

During NieR's prologue, Yonah and Nier hide in an abandoned supermarket from attacking relapsed Gestalts. After dispatching three of them, Nier checks in on Yonah. He reassures her that he'll protect her, returning to the entrance only to find more enemies. Nier fends them off before being knocked to the ground. Overwhelmed, he decides to give in to a copy of Grimoire Noir previously given to him, trading his soul for power. Nier fights dozens of Gestalts using the book's power, easily killing all of them.

When he hurries back to Yonah, she offers him a cookie. Yonah starts uncontrollably coughing, and strange markings begin traveling up her body. She reveals that she also touched the black book in an attempt to help her brother, but her body rejected its power, causing her to to relapse.

The Lost Verses and the Red Sky adds that Yonah fell into a coma shortly after this occurred. Finding a cure for her immobile state was the sole motivation for Nier agreeing to become the Shadowlord. However, the scientists behind Project Gestalt were unable to do so, and even Nier's power as the Original Gestalt could not cure or help her. It would have been easier for her to die, but to avoid jeopardizing the entire Project, her Gestalt was instead placed in a cold sleep. Her Replicant's creation was to ensure that her soul would have a body to return to after the thousand year wait.
